This month we’d like to hear from you, your relatives, carers or friends, about experiences of hospital outpatient appointment letters. If this does not affect you, please ignore this email.

- Was the information in the appointment letter clear and easy to understand?
- How satisfied you are overall with the communication regarding your appointment.
- Any suggestions you might have for improving the appointment process.
