Diabetes Grants Programme
ESCV managed a diabetes grants programme on behalf of Sussex Health and Care.
In 2023, ESCV was commissioned by the Sussex Health and Care Diabetes Clinical Network to manage and monitor a grants programme, supporting four projects from the voluntary, community and social enterprise (VCSE) sector with reducing health inequalities in diabetes care.
The programme aimed to trial new partnership working approaches to support the development of an integrated model of diabetes care, and to improve outcomes for people living with or at risk of type 2 diabetes.
What did we do?
For ESCV, managing the grants programme included administering grant payments and agreements, liaising with the four funded projects, and reporting back to the clinical network board.
Grant funding was provided to two homelessness charities, one community project, and one GP practice working in partnership with a VCSE organisation.
At the start of the funded period, we visited each project to build connections and establish a relationship, and we worked with them to set milestones and agree reporting timeframes. As the project progressed, we kept in touch with each project via regular in-person meetings, calls and emails. We also visited project activity to see their work in action where possible to do so.
As we got to know the projects, we were able to provide guidance in areas where they needed more support, such as how to evaluate and report on their project activity. Having liaised with the funder on the impacts they would like to see reported, we created bespoke evaluation guidance and reporting forms that would support projects in being able to demonstrate these impacts and their learnings from the project work.
We also arranged a training and networking day for the four projects, so that they could access training from Diabetes UK and share ideas and best practice with each other. This was well-received by participants, with 100% rating it as either ‘excellent’ or ‘good’.
Across the year, ESCV worked to monitor and mitigate project risks, and to liaise with the funder in relation to the programme. Regular meetings with the funder took place across the project period and, as the projects progressed, ESCV provided three written reports and presented four updates to the Diabetes Clinical Network meetings between July 2023 and September 2024.
What was achieved?
The four funded projects successfully completed their work in spring 2024.
ESCV compiled a full evaluation report for the programme, detailing the activities, impacts and key learnings from the four projects to share with the Diabetes Clinical Network.
In the final report, we drew on the learnings and impacts of the four projects to identify 30 recommendations. These included 10 recommendations for GP practices, 16 recommendations for NHS Sussex, and 4 recommendations for VCSE organisations.
